Overview

PBM stands for Photobiomodulation; formerly, it was also known as Low-Level Light Laser Therapy (LLLT). PBM is the use of light to therapeutically treat the body, and benefits can include tissue repair, the reduction of inflammation, and increased energy production.

While some may be skeptical of light’s healing properties, we must remember that light is highly connected to the various human organ systems. For example, the sole fact that you are able to read this post is because light is causing photosensitive cells in your eyes to send electrical signals throughout your brain. We also use the sun to produce Vitamin D when we absorb its radiation into our skin. 

But how does it really work?

The reason why we need to breath oxygen and eat food is because they allow the “power houses” within our bodies’ cells (small organelles called mitochondria) to generate ATP, the molecule that ultimately gives our bodies energy. During this process of converting oxygen and food into ATP, a specific enzyme called Cytochrome C Oxidase (CCO) is in charge of binding oxygen into this process; however, free radicals and other molecules (such as ROS, or Reactive Oxygen Species) can sometimes compete with Oxygen and take its spot, which “clogs” ATP generation in the mitochondria. This issue is referred to as Oxidative Stress, and it is one of the causes of cell death, early aging, and many other health problems including cancer. PBM, using light waves, excites CCO and drives away ROS, allowing more oxygen to come back into the ATP-making process. 

This edge cutting technology has been studied and scientifically proven to affect our bodies through a range of benefits. As mentioned by Pinar et al, PBM can be used for stimulation of healing, reduction of inflammation, reduction of cell death, and skin rejuvenation.

1. Decrease Inflammation

PBM has been proven to diminish the expression of inflammatory proteins and improve blood circulation in the body. According to a study carried out by Michael Hamblin, one of the most reproducible effects of PBM is an overall reduction in Inflammation, which is particularly important for disorders of the joints, traumatic injuries, lung disorders, and the brain. ROS are necessary for many chemical reactions in our body, including apoptosis (cell death). Typically, ROS are balanced inside our cells by antioxidants such as Glutathione. If ROS and antioxidants become unbalanced, harm can come to our bodies. PBM “can up-regulate antioxidant defenses and reduce oxidative stress” (Hamblin).

Risks and side effects

There are no known side effects of using PBM Red Light Therapy because it uses low-energy wavelengths that do not have enough energy to remove electrons from atoms (non-ionizing radiation). However, caution should be used when exposed to high-capacity lasers. Potential hazards in these lasers include burning, soft tissue damage, and blindness. What is Osteoarthritis?

Arthritis is a general term that means inflammation of the joints.

Osteoarthritis, commonly known as wear and tear arthritis, is the most common type of arthritis. It is associated with a breakdown of cartilage in joints and can occur in almost any joint in the body. It commonly occurs in the weight-bearing joints of the hips, knees, and spine. It also affects the fingers, thumb, neck, and large toe.

Osteoarthritis is the second most common condition associated with years of living with disability, just behind diabetes. Pain often drives people to the doctor, and it’s important to seek treatment as early as possible to avoid further joint damage and to alleviate your pain for the long-term.

Symptoms of Osteoarthritis

  • Osteoarthritis symptoms often develop slowly and worsen over time
  • Symptoms tend to build over time rather than show up suddenly. Swelling & Inflammation around a joint.
  • Pain or aching in the joint during activity, after long activity or at the end of the day.
  • Joint stiffness usually occurs first thing in the morning or after resting.
  • Limited range of motion that may go away after movement.
  • Clicking or cracking sound when a joint bends.
  • Muscle weakness around the joint.
  • Joint instability or buckling (knee gives out).

Who Gets Osteoarthritis?

Osteoarthritis affects an estimated 27 million Americans. The chance of developing the disease increases with age. Most people over age 60 have osteoarthritis to some degree, but its severity varies. Even people in their 20s and 30s can get osteoarthritis, although there is often an underlying reason, such as joint injury or repetitive joint stress from overuse. In people over age 50, more women than men have osteoarthritis.

Cryotherapy for Osteoarthritis Pain Relief

In the study of 40 patients suffering Osteoarthritis, each received 10 Cryotherapy treatments and were asked to rate their pain, the frequency of the pain, and their use of pain medication before as well as following Cryotherapy sessions for pain management.

Of those who completed the study, 80% of the Osteoarthritis respondents recorded that following their Cryotherapy treatment there was a significant improvement in the reduction of pain, with either zero pain recorded or very infrequent periods of low level pain.

Prior to the Cryotherapy pain management treatment, 48% of the patients reported strong pain that was either periodic, often, very often or continuous. Another 43% reported a mild pain of the same nature, and 5% recorded very strong pain.

Cryotherapy for Osteoarthritis Pain Relief

Following 10 sessions of whole body Cryotherapy 25% of people recorded no pain at all (previously all 40 study patients recorded some level of pain) while 63% recorded mild pain only periodically, or often following the study, a reduction in the level and frequency they started with prior to Cryotherapy treatment. All those in the study also reduced the amount and frequency of pain medication with 85% able to go without their usual pain medication and 10% able to take only on the spot painkillers for those infrequent moments of pain.

More importantly, mobility increased. With those who required partial assistance before the study then able to gain their independence back, an additional 23% of those who had limited mobility before Cryotherapy pain treatment reported no limitations.

The study was focused on the wellbeing of the patient and was looking for a proven way to allow physical activity, increased relaxation as well as an overall improvement in general well-being in those suffering from the pain and limited movement caused by Osteoarthritis.

Infrared Sauna for Osteoarthritis

One of the most effective treatment methods for osteoarthritis is heat, and this is exactly why Far Infrared Sauna Therapy (F.I.R.S.T.) is one of the best ways to help reduce the symptoms of osteoarthritis. What is Ankylosing Spondylitis?

Ankylosing spondylitis (AS) is a rare type of arthritis that causes pain and stiffness in your spine. This lifelong condition, also known as Bechterew disease, usually starts in your lower back. It can spread up to your neck or damage joints in other parts of your body.

Case Study #1

 

 

Background of Study:

The aim of this study was to assess the impact of whole-body cryotherapy with subsequent kinesitherapy on spinal mobility parameters in patients with ankylosing spondylitis. Material and methods. We enrolled 32 men with ankylosing spondylitis in a clinical trial. The subjects were randomly divided into 2 groups consisting of 16 persons, with no significant differences in age, duration, or stage of disease, treated with a cycle of 10 whole-body cryotherapy procedures with subsequent kinesitherapy or kinesitherapy alone, respectively. Routine spinal mobility parameters were determined for all patients before and after the end of the therapeutic cycle.

Results:

Significant improvement of spinal mobility was observed in both groups of patients, but in patients exposed to whole-body cryotherapy with subsequent kinesitherapy the percentage changes in the values of particular parameters were more distinct as compared to patients in whom kinesitherapy alone was used, mainly in respect to lumbar and thoracic spinal mobility. Conclusion. The use of whole-body cryotherapy as a component of comprehensive therapy in patients with ankylosing spondylitis produces significant improvements in spinal mobility parameters as compared to patients in whom kinesitherapy alone is used.

Case Study #2

 

Background

The treatment of ankylosing spondylitis (AS) patients requires a combination of non-pharmacological (education, exercise and physical therapy), as well as pharmacological treatment modalities. The optimal management of AS still remains unresolved.

Objective

The aim was to measure and compare the effects of whole-body cryotherapy (WBC) at -110∘C and at -60∘C and exercise therapy alone on disease activity and the functional parameters of patients with AS.
Methods: Ninety-two patients were allocated to three groups: with WBC at -110∘C or at -60∘C (each concurrent with exercise therapy), or exercise therapy alone. Disease activity and the functional parameters of the patients were measured at study entry and at the end of the 8-day treatment.

Results

Supervised therapy, irrespective of the program, led to a significant reduction in disease activity (Bath Ankylosing Spondylitis Disease Activity Index: BASDAI, Ankylosing Spondylitis Disease Activity Score: ASDAS-CRP), disease-related back pain, fatigue, duration and intensity of morning stiffness and a significant improvement in the patient’s functional capacity (Bath Ankylosing Spondylitis Functional Index: BASFI), spine mobility (Bath Ankylosing Spondylitis Metrology Index: BASMI) and chest expandability, with no changes in the levels of CRP. It has been demonstrated that following therapy, the group that underwent cryotherapy at -110∘C manifested significantly reduced disease activity (BASDAI) compared with exercise therapy only (p= 0.024).

Conclusion

Adding cryotherapy at -110∘C to exercise therapy led to significantly reduced disease activity expressed in BASDAI, compared with exercise therapy alone.

What is Rheumatoid Arthritis?

A chronic inflammatory disorder affecting many joints, including those in the hands and feet. In rheumatoid arthritis, the body’s immune system attacks its own tissue, including joints. In severe cases, it attacks internal organs. Rheumatoid arthritis affects joint linings, causing painful swelling. Dr. Yamaguchi of Japan who first started using freezing treatments of short duration on his Rheumatoid Arthritis patients. He found that a raid decreas in temperature on the out layer of skin (epidermis) leads to an immediate release of endorphins causing people to become less sensitive to pain.

Cryotherapy Case Study

The aim of this article was to review current evidence about cryotherapy in inflammatory rheumatic diseases(therapeutic and biological effects). For therapeutic effects, we performed a systematic review (PubMed, EMBASE, Cochrane Library, LILACS databases, unpublished data) and selected studies including non-operated and non-infected arthritic patients treated with local cryotherapy or whole-body cryotherapy. By pooling 6 studies including 257 rheumatoid arthritis (RA) patients, we showed a significant decrease in pain visual analogic scale (mm) and 28-joint disease activity score after chronic cryotherapy in RA patients. For molecular pathways, local cryotherapy induces an intrajoint temperature decrease, which might downregulate several mediators involved in joint inflammation and destruction (cytokines, cartilage-degrading enzymes, proangiogenic factors), but studies in RA are rare. Cryotherapy should be included in RA therapeutic strategies as an adjunct therapy, with potential corticosteroid and non-steroidal anti-inflammatory drug dose-sparing effects. Infrared Sauna Case Study

To study the effects of infrared (IR) Sauna, a form of total-body hyperthermia in patients with rheumatoid arthritis (RA) and ankylosing spondylitis (AS) patients were treated for a 4-week period with a series of eight IR treatments. Seventeen RA patients and 17 AS patients were studied. IR was well tolerated, and no adverse effects were reported, no exacerbation of disease. Pain and stiffness decreased clinically, and improvements were statistically significant (p < 0.05 and p < 0.001 in RA and AS patients, respectively) during an IR session. Fatigue also decreased. Both RA and AS patients felt comfortable on average during and especially after treatment. In the RA and AS patients, pain, stiffness, and fatigue also showed clinical improvements during the 4-week treatment period, but these did not reach statistical significance. No relevant changes in disease activity scores were found, indicating no exacerbation of disease activity. In conclusion, infrared treatment has statistically significant short-term beneficial effects and clinically relevant period effects during treatment in RA and AS patients without enhancing disease activity.
